Mail 3 UNFOLDED sheets of your letterhead to:  Follow-Up Plus, PO Box 294, Carthage, IL 62321

Or if possible email it to:  followup@frontiernet.net

Or ask us to design letterhead for you.  A business card or website can be helpful if you use a specific logo that you want on a letterhead.

Vehicle Purchase Invoice (Bill of Sale) - needs

Nickname if different than given name

Date and month of birth for both buyer and co-buyer

Name of purchaser if buying for a company

Salesperson's name

Fax the Vehicle Purchase Order (Bill of Sale) to Follow-Up Plus at 217-357-9076
